Welcome to the practice.  We are looking forward to meeting you soon.  Please bring your photo ID, your insurance card, and method of copay (if applicable) with you to your first appointment.  

All New Patients:  To speed up your time in the waiting room please do the following:

1) Register for our patient portal (CLICK HERE) and select the “sign up” option if you are new to our portal

2) Two (2) days before your appointment a “self check in” and health history questionnaire will appear on your portal. Please fill in this information prior to arrival.

** PLEASE LOG IN WITHIN 48 HOURS OF YOUR APPOINTMENT TO CHECK IN AND FILL OUT YOU HEALTH HISTORY INFORMATION. THE LINK TO CHECK IN WILL NOT APPEAR UNTIL 2 DAYS BEFORE YOUR APPOINTMENT**

3) If you can also please print and sign this HIPPA Policy and Sign the Authorization and Release form.

**Please Note: Our patient portal is unique to our practice, we are not on the same network as Abington/Jefferson.

*If you have concerns about your sneakers/shoes or existing orthotics please bring them with you to your appointment*

*If you have an issue with your toenails, please remove your nail polish*

Diabetics:  All Diabetic patients please additionally fill out this New Diabetic Form along with general "New Patient" form/portal above and bring with you to your first visit.

Athlete/Runner/Sports Injury:  If you have an athletic or running related issue or a sports injury please fill out this additional Form as well. If you are a runner with a running injury, Please bring your Running shoes with you!
